The ABC (Action on Diabetes, Blood Pressure and Cholesterol) is a therapy optimisation clinic run by senior pharmacist Natasha Jacques who specialises in diabetes and Melanie Patel a diabetes specialist nurse. In this clinic an in-depth review of the patient’s current treatments is given, looking at specific targets for blood glucose, blood pressure and cholesterol. The clinic is run on a Monday and Thursday morning, and can take referrals from the general diabetes clinics.
