Patients with complex diabetes management issues are referred from clinic, the wards and community. Patients may also self-refer. Difficulties in self-management are explored from the patient’s perspective. An agreed action plan is negotiated with a view to minimising short and long term health implications of diabetes.
Clinics are held in the Diabetes Centre at Heartlands Hospital every Wednesday and Friday morning.
